Note: Make your first appointment before you register. Please call 812-244-0400.
THEN, complete the registration process:
For each adult to be counseled: complete the Adult Registration: This can be done online, or by arriving 20 minutes early for your first appointment.
For minors (age 17 or less), a parent or guardian completes the Minor registration.
After making your appointment by calling our office, please click the appropriate button below with the name of your counselor to complete the registration process.
